I think it's great to share forms and ideas with one another. Sometimes being so swamped with other task, it's easier to just send the form out and be done with it. And everyone can just tweak it to fit their needs.
OK, here is a question. What would an easy to use, customizable, and secure software application like this be worth to parent groups? And, yes, I know that we might literally be "stealing from the mouths of babes." I think that I can get someone to work on something like this, but I'm sure they'll want to know "what's in it for me?"
