For all...something to think about if you conduct a survey: Don't ask if you don't want to know. In other words, if you ask people for their input, they have a right to expect you will act on their comments. If your board really doesn't intend or have the time to respond to the comments and criticisms that will come in the survey responses, you might be better off surveying informally (by just talking to people). You really don't need someone complaining publically, "I told THEM about xyz, but they never did anything about it!"
Surveys can be a valuable tool, but they can cause a huge "follow-up" burden. Just think carefully as you craft your questions.
