I have always used Excel to create my order forms. Sending you what I have probably wouldn't help much because I'm sure what your school is offering is different then what we have. I will try to make it more generic and see if I can download it to the site. I can give you some thoughts to:

We do prepaid orders and at the top of the form I state that the form must be accompanied by full payment. I also explain how/when the order will be filled. Also make sure that you get the student's name and homeroom and the parent's name and a phone number in case there are any issues. I do mine with the item listed and its' price and next to it I put the available sizes with a line next to the size where the customer can place the quantity they want of that size. I then have lines for subtotals for each item and a total at the bottom of the form.

I'll let you know when I have downloaded the form.

Our school has decided to go with a spiritwear company that will supply free order forms for the studenst but their customer service is not that great. We found a company that we think we like better but we will need to make our own order forms and dont know how or where to start. Is there someone that has a form that they have come up with and are willing to share or at least give us a general idea on what it should look like?
