Renaissance Academy will be putting together a Family Directory to be distributed to all Family School Organization (FSO) members in October 2005. The following information will be included in the directory:
 Teachers, grade, phone extension and email
 Class List for each grade
 Student name, grade, address, phone number and parent name
If you do not wish to have your child’s address and phone number included in the Directory, please send back this form to your child’s teacher by September 16, 2005.

Remember that it's in Access as that is the only type of program which will work for this application....no, I'm wrong, Filemaker would work as well. This program could be easily converted if you are a Mac user like myself.
