Here's what I made up when I was President this past year. I used the free survey service at
www.zoomerang.com, and emailed all PTO members who had given me their addresses. For those members who did not provide an email address, I printed out copies, and sent them home via the "backpack express".

Out of 172 surveys sent, I got 88 replies--not too bad. I tallied the results, and then let everyone know the top 3 vote-getters in what to do with the money, and the top 3 fundraiser ideas. We then worked hard to accomplish as many of the members' top choices as possible. (I hope this info lines up clearly; sorry if it does not).
