Our directory address section begins as a tab delimited text file from our school's computer system. Ultimately our directory chair converts it to publisher. We tried using a database like Access, but we have so many exceptions to the standard layout, that it was easier to use a more flexible tool like word or publisher.
Our PTO produces the student directory which we then sell to school parents for $10 each. Most people buy the directory. We also sell advertising so it's a decent fundraiser ($1,500).
We don't have to get permission from the families for the data to be included, but we do ask for permission to publsih unlisted phone numbers. Also, the non-traditional families (two sets of parents, two addresses, etc.), need to contact the directory chair since that information isn't always in the school's computer system. Otherwise, we're free to include everyone. We have a statement on the directory about not using it for commercial purposes. So far we haven't had a problem.
In additon to student/parent names, addresses, phone, we also include class lists, a list of staff, key dates for the year, PTO officers, school board members, key school phone numbers and business advertising. The cover is selectd from drawings submitted by 5th graders (the oldest grade in our school).
We also do a directory, but at our silent auction the prior school year we auction off the back page of the directory to two businesses and that pays for the directory that we offer to each family at school for free!

Does anyone know of a program for entering directory data names, numbers etc? I have looked for one off the shelf but haven't found anything catered specifically to directories.

If you do, I have developed a program where I manage all the things you say you want and automatically places the data in a nice report for printing.
Typing is minimal after the 2nd year, as update querys change the childs grade and only new students and kindergarteners need to be typed.
