Your form sound like the survey we send out at the beginning of the year. It helps us get to know new parents.
We don't have a form or application for nomination because by the time a person decides to run for office, we know a lot about them already.
Basically, our form just said "we'd like to get to know you better, please tell us a little about yourself"
"Your child(ren) at school, do you work, if so part time/full time and what type of job do you do? Are you involved with Scouts or your children's sports?z'
It was that simple and the nominee's came back with lots to tell about themselves which really helped with the voting.
I'm sorry, I didnt save the original form that I had created, but this was basically what it said.
We just send out a form accepting nomination, then we notify those who have been nominated to see if they want to accept or not.
This year, we did a short interview-bio form they filled out basically telling about themselves i.e. involved in scouts/sports, work pt or ft, who their kids are and so on.
We then typed up these bios and attached them to our monthly newsletter.
Many thanked us for doing it as every year people tend not to vote as they have no idea who these people nominated are.
