Hi - I was wondering if you would be so kind to mail me a copy of your reimbursement forms. They seem like something I could definately use. I am the PTO president for our Middle School for the new year and the Secretary for our Elementary School we are in the process of making changes in both schools and these forms would be great. Sue - your original post asked for a reimbursement form, so I will email you the one we used this past year. The file is in WORD and includes 3 other forms as well (deposit, check request, and cash box request). I copied each on different colored paper. We, too, included one in each committee chair packet, plus we kept a supply in the PTO mailbox. Seemed to work pretty well.

Thanks for the link to the Utah volunteer form. It's much more detailed than we need, but I love the variety for areas of involvement.

DCD: Yes, there are lots of versions and these seem pretty popular. Here's another one - probably some overlap with what you found.

Volunteer Survival Kit

As a new volunteer, here are a few things to help you "survive" the experience!

Toothpick -- to remind you to pick out the good qualities in others.

Rubber band -- to remind you to be flexible, things might not always go the way you want, but it will work out.

Band Aid -- to remind you to heal hurt feelings, yours or someone else's.

Pencil -- Pencil - to remind you to list your blessings everyday.

Eraser -- to remind you that everyone makes mistakes, and it is okay.

Chewing Gum -- to remind you to stick with it, and you can accomplish anything.

Mint -- to remind you that you are worth a mint.

Candy Kiss & Hug -- to remind you that everyone needs a kiss or a hug every day.

Tea Bag -- to remind you to relax daily & count your blessings.
