This is what we used:

Part VIII – Your Specific Activities
Section 4a - Planned Fundraising Programs

XXX Middle School PTO annually conducts 3-4 major fundraising projects to raise funds for programs and activities to support the school such as the following:

1. Gift Wrap Sales – annual catalog fundraiser
2. Pie and Cookie Dough Sales– annual catalog fundraiser
3. Market Day – monthly fundraiser
4. Book Fair – annual fundraiser

All fundraising materials are sent home through students of the school.

There are sections that want you to explain your fundraising events and what events you have during the course of the year. They state that you should have supporting documentation should they be in any way related to Bingo, raffles, etc. We have BINGO, but admission is food for our local food pantry. We also have a Ladies Night Out which raises money for expenses the 2nd part of the year. So we need to attach something.

What sections are you referring to? I can send you the text we used for our application.

I was just wondering how much supporting documentation I should be sending in with my application and how much you submitted with yours?
I have the financial information to send, but to support your explanations of fundraisers, events and the like, how much did you include.

I want to avoid any/all delays and hopefully get it approved the 1st time through! Your advice would be greatly appreciated
