Panera does, and you can simply fill out an online form

PinkCalyx.com donates $50 gift certificates to all non-profits, in exchange for a LIKE on their facebook page. The procedure is outlined on their website:

pinkcalyx.com/charitygiftcertificates.aspx

Once they receive your request via their website, they email you a printable certificate.

Mail request are no longer honored.
