We made up our own sheet. People could donate a flat rate or per word. Most did flat rate. The words used where that week's spelling words. The donation sheets were turned in on Wednesday and on Friday the sheets with the test attached were sent back home with the child. The money was due this past Wednesday (we are still having money sent in). Our kindergarten class who do not have spelling tests were tested on colors and shapes...
So say 4th grade had 20 words. If someone donated 10 cents a word that person would owe $2.00. But like I said most people did flat donation which made it easier on the parents.

Hello everyone! We just completed our fall fundraiser. The proceeds are going toward the purchase of a new copy machine for our teachers. With little effort and no middle man, we made $4225. We are a small school of 156 children and we sadly only had 70 kids participate, I felt it went great.
Each participant received a water bottle with the school name and logo. The top donator in each class won a $15 gift certificate to Wal-Mart and the top donator in the entire school won a bike.
