Question: Help with the 501(c)(3) form

Who else has filled this application out and how did you do it? We're having trouble figuring out what we're supposed to say. Help!

I filled out ours for our Parent Group this year - started in January finished in July - it is a lot of work and a lot of repeating yourself, best bet is to get the PTO Today guide for this - you have to be a plus member or purchase it for about $40 or so - it is money well spent!
