Hello! Does anyone have a form they use with teachers so that they can easily request funds from the PTO? Our old PTA had a very complicated form that deterred teachers from going through the process. We are looking for a form that provides the basic information we need, without too much red tape.
Any help would be appreciated! We would love to see some sample forms that work for your PTOs. I tried looking in the form exchange and searched "Teacher fund requests" or "fund request forms" but couldn't find any.
