We don't currently have a form like that on our File Exchange. But we like the idea of including some important information on one sheet and are looking at ways to create a document for our File Exchange. One suggestion is to keep password information separate from information like this so you keep the group protected.

I'm wondering if anyone has an "important info sheet". Something that lists out EIN, Bank info, passwords, ect...so it's all in one place. Would also help with officers transitions.
