I posted a teacher survey in Word form in the file exchange. You can edit it to tailor it to your group. I just received the first one back from a teacher after only a couple of days and the answers were already very helpful

I sent this to my son's teacher and assistant teacher and got the information I needed for classroom needs (material and volunteer needs) and what her hobbies/interests, favorite places to eat/shop were. This should make holiday gifts/teacher appreciation easier and more personal.

Check out SurveyMonkey.com. You can put together online surveys quite easily.

I am trying to put together a survey for our teachers to find out what they need from us, what we can do for them, and to let them know how they can help us. Anyone have any ideas or a form for this already that I may be able to alter?
