I have to say that having used forms and folders I much prefer the folders option. It is much less time consuming. We take 2 tables and lay out manilla folders that on one side list the event with a description and on the other side has a sign up sheet for emails and contacts. At the end of orientation night we simply fold up the folders and pass them to the chairperson to contact the interesed people. You need to man the tables but the resulting tool is much easier than shifting through hunderds of membership form entries.

I am a newly elected chairperson for our Parent/Teacher Organization. I am looking for a parent volunteer sign up sheet that asks for help as well as a one-time family participation donation. Anyone out there have an idea or sample I can look at?
