I don't know if this will help you, but at our Open House we put two forms out in each teachers room. One is for Room Parents the other is for Party Helpers. The teachers place the sheets beside their sheet that they have the parents fill out for child information so that they are not over looked. On the top of the page in big letters we write "Room Parent" at the top of one and "Party Helpers" on the other. We give a description of the duties for each and then underneath is a place for the parents to write their names, childs name, and phone #.
