We also did an "opt-out" form. Anyone who did not want their information included was left out when it went to print. The deadline for turning in your "opt-out" form was about 2-3 weeks before we went to print, so the stragglers had every opportunity to get out of the book. We charged $2.00 for the directory, and our cost was $1.76 or so - not a fundraiser, just a service thing and I can't tell you how many times every day I reach for that thing.
We have a directory and it is optional. You can request to NOT have your info in there. As long as the directory stays with parents/family of students, it shouldnt be an issue.

As for getting parents involved - keep on reading. I would say have someone from PTO (a different person each time) show up at ANYTHING at the school and talk to everyone they can about the great things that the group does for the school and how those things wouldn't be possible without the PTO but that the PTO needs more volunteers. Also - if you have a school directory - start calling! Call parents and invite them to the next meeting. Have the committee chairpeople call and ask for volunteers for an upcoming event. Be sure to include any new families to your school, also - ask at the office if you can receive the name and phone number of new students and then call those parents to let them know what's going on in PTO, when the next meeting is and ask how (not IF - how) they would like to get involved. The personal touch will help.
How to get businesses to donate - well, that takes the personal approach as well - knock on doors and ask, either send a letter first or after your visit if they haven't given you anything yet and don't forget the THANK YOU!
