If you look in the bonus tools, there is a reader submitted donation receipt there that you could modify to do what you need. You could send a copy of the form back with the thank you letter.
Thank you notes are SO important! We are stanting a new Holiday Basket Auction at our North Pole Family Event and need to track the donations. My chairperson is fantastic but not very computer friendly (her husband typed the letter) so I need a manual form for her to track the contact person, telephone number, name and address of companies that she is contacting. Also what was donated, what it was valued at and a check off that she has sent a thank you letter from our PTO. (Did I forget anything???)
