Since this activity has very customized aspects to it, you won't find any standardized form for it. You'll just have to make a list of everything that has to go on the form taking into consideration historical experiences your group has had, write it, look at it from the perspective of someone who is completely new to the process, then revise it to make the instructions crystal clear and understandable.
With regard to the issues that arise in the end, it sounds to me that your board should consider an alternative process that would eliminate the potential for stand-offs with parents in the end about payments due. In other words, figure out a system where the parents don't get the products or tickets up front and you'll have a solution to this repeating problem.

Does anyone have a form they would share that introduces a fundraising event, the rules, and outlines the responsiblities of the person accepting the
tickets or product?
I am looking for a letter of some kind that I can customize for my school. I need a letter that would explain the fundraiser, who, what when, which would let the person know that it is voluntary, however if they accept the tickets or products they are responible to return the money or tickets/product by a certain date. If the ticket/product are not receivevd the person becomes responsible for the payment.
I'm hoping to alleviate some of my frustration when parents suddenly appear on the final day that the product/tickets must be returned saying that they have lost or misplaced them. That would be ok if they resumed responsiblity for payment, but some of them actually feel they should not have to pay. I find it amazing that the tickets sometime show up at the event, (remember they were not paid for). I have sought the permission of the principal to charge the payment to the tuition of students.
This is why I would like to see what others have come up with. With fair warning I'm hoping to cut down on the sad, irresponsible tales I have to hear.
