I wouldn't know how to the ballot - we've never actually needed one (we only have one person at a time running for a position at once)

I am parliamentarian. My first year. We are having elections in May and I need a sample ballot and notice - describing each office - to send to membership. Also do I need to list current officers in the notice and on the ballot? Please help.
