I was pleased to hear there is a survey on the file exchange but I can't find it. I am new to this site and I have been looking.

We are curently trying to create a parent survey to ask parents what types of family event they would like to attend this year. Does anyone have a template for this type of survrey? Does anyone have experience sending out parent surverys?
