What you collect will obviously depend on what you are trying to achieve.
One idea we had was to collect some basic information from each teacher and create a small poster to be placed beside their door with a digital photo and the info – mostly background, but with a few zany trivia questions like ‘favorite flavor of ice cream†or “favorite character from a bookâ€. We thought these would be fun especially at the beginning of the year and during parent night. Unfortunately, the project never crystalized, but idea sounded fun.
Sometimes you can do something similar - profiling a teacher in a newsletter. But it's hard to get to everyone unless you do several at a time.
Again, what you choose to ask depends on the purpose, how much space you have, the format, what teachers are willing to share. But here are some ideas:

Honors/Awards/Grants
Where born/where lived previously
Activities & Hobbies
Favorite ______
(whatever you want to ask: color, music, TV show, zoo animal, book, character, restaurant, type of food, store....)
Trivia Questions - again, just however creative you want to get. For example:
1) What do you think you'd be if not a teacher?
2) Most adventurous thing you've ever done
3) Favorite subject when a student
4) Least favorite subject when a student
5) If you had to be an animal which one would you be?
