Teacher Personal Questionnaire for Parents

9 years 11 months ago #166588 by Rose H
Hi Heather,

Wanted to pass along a few resources to you. We have several teacher survey templates on our File Exchange that you may find helpful. Here's the link: www.ptotoday.com/filesharing/category/82-teacher-survey

Also, noted that you are a room parent. We just launched a new site dedicated to home room moms like you! Here's a link to the new site: www.homeroommom.com.

Good luck and stay in touch!

Hello,

love the idea of this - just starting out with our daughter starting Kindergarten and I am a Room Parent. Would you be so kind as to share the completed questionnaire? thanks so much!

I would love to have a copy of this questionnaire. I am the volunteer coordinator at my kids school and I know our parents would love this information. This email address is being protected from spambots. You need JavaScript enabled to view it.

What you collect will obviously depend on what you are trying to achieve.

One idea we had was to collect some basic information from each teacher and create a small poster to be placed beside their door with a digital photo and the info – mostly background, but with a few zany trivia questions like ‘favorite flavor of ice cream” or “favorite character from a book”. We thought these would be fun especially at the beginning of the year and during parent night. Unfortunately, the project never crystalized, but idea sounded fun.

Sometimes you can do something similar - profiling a teacher in a newsletter. But it's hard to get to everyone unless you do several at a time.

Again, what you choose to ask depends on the purpose, how much space you have, the format, what teachers are willing to share. But here are some ideas:

Name
Grade/Subject
Graduated From
Degrees
Years of Experience
Where else/what else taught
Honors/Awards/Grants
Where born/where lived previously
Activities & Hobbies

Favorite ______
(whatever you want to ask: color, music, TV show, zoo animal, book, character, restaurant, type of food, store....)

Trivia Questions - again, just however creative you want to get. For example:
1) What do you think you'd be if not a teacher?
2) Most adventurous thing you've ever done
3) Favorite subject when a student
4) Least favorite subject when a student
5) If you had to be an animal which one would you be?

Please share how some of you might use a teacher survey. It's always fun to hear about projects like this.
