We have a program in my county that provides mentors (usually mothers or grandmothers) for teenage mothers. I was going to become involved, but became too active in PTO to do that too. It is sponsored by this county's Prevention Partnership office. This is a non-profit organization trying to help kids find non-drug related activities. In our county this is the group that sponsors the high school post-graduation all-night parties. You may have something like this in your county that could help you.
Unfortunately, this is an area that I know very little about. I hope I didn't sound nasty when I asked all those questions. That was not my intention. I really did not understand exactly what you are trying to do and what type of info will help you do it. When I mentioned all the "wonderful folks" here who could contribute info, I was NOT referring to myself.
