Hey--I've been thinking about how to do this and I did one time send in a funny email to the music teacher who sent home recorders the Friday before a long weekend. I broached it like I was writing from the funny farm. Perhaps you could use the same approach

Dear Teachers;
Summer is over and before I leave with the very nice men that were sent by the very nice doctor my husband took me to last week I wanted to get this survey out. Oh, don't worry about me...just a long summer with the normal bumps and bruises associated with the children being home. Not sure how you do it all year but the doctor I have says that after a very nice week or so at Sunnydale I should be back to normal in no time. Just wanted you to know how much we appreciate what you do every day (my goodness the birdies are singing nicely today don't you think?). So when you get a chance this next couple of weeks, if you could fill this out and return it I'm sure I'll be able to sort through everything once the padding is removed from the room I'm staying in.
Thanks, and sorry this is written in purple crayon...it's all they'll let me use right now.

With great appreciation for your chosen profession;
____________ PTO

Anyway maybe that'll give you some ideas

I want to enclose a welcome back letter to our staff with our yearly survey. I am thinking something funny.. like "we can't wait to share our children with you" (b/c they are driving us CRAZY). I tried to find something online but they were all letters FROM teachers. Thanks for any help!!!!
