I agree. It does not need to be formal. Just the position, nominees names, and a place to put a check mark. You will need two members who are not nominees to collect and count the ballots and report the results. Then ask for a vote to destroy ballots.
We usually have only the name on a ballot and they are listed in alpha order. That way no one questions the order listed:

You get the idea. . . . You may want to also indicate how many people may be voted for in each group. Some put a check box next to the name, others just have you circle the person's name.
Does anyone have a sample ballot or know where I could find one? We have had a board member resign due to medical problems and we are having an election for her replacement. As a first-time pres. I've never had to run an election. I'm not sure exactly what needs to be on the ballot. Do each candidates' experience go on the ballots, or just their names?
