I would call the IRS and apply over the phone and ask if you need to provide a personal SSN. The instructions say you need to provide the name and SSN if the entity (PTO) is a corporation. If you haven't become a corp yet, then I'm not sure you need to give them anything in line 7. I applied over the phone and the form I referred to when I called them has those two fields blank. In other words, I don't think I gave them a name or SSN. I would certainly verify the rules before I linked my SSN to the PTO's EIN for eternity. 1-800-829-4933
If you call the IRS, they will take your EIN application information over the phone, then will give you your EIN number immediately. They will follow up by mailing your EIN number to you.
WE have switched from PTA to PTO. I faxed the form to apply for new EIN two weeks ago. My question is: Has anyone applied for their EIN and how long did it take to come? Thanks
