Question: Parent requesting donation receipt for monetary donation
Our yearly fundraiser is a walk and fitness day. A parent has asked for a donation receipt. I found the donation receipt on this website and am hopeful that I can use it for this purpose. However, it looks like this is a receipt for a community member/business. Can it be used for a personal donation as well? And if so, why is there a line that asks the parent to return the form back to the PTO? Does the PTO need it for some reason?

PTO Donation Receipt
Also, just a note that your PTO must be registered with the IRS as a charity under section 501c3 of the federal tax code in order for donations to be tax deductible. If your group isn't a 501c3, then the person making a donation to your group cannot deduct it from their taxes. They can, however, make the deduction directly to the school. In that case, the receipt would need to come from the school, however.

We are a 501c3 so good to know this person can get the donation deduction. however, I'm still not sure why the form says to return it to the PTO, is that necessary or is the receipt for their records only?

Craig writes:Good question. I think that the idea was that in some cases the PTO needs a record as well (thus the signature line for the donor). But it's unlikely you would leave the form with the donor and ask him to send it back.
I deleted that line from the form and reposted the file. If you click that link again, you should get a version without that line.
