This form explains that the PTO puts together a student directory each year. It asks parents to complete the form with information so their student can be included in the directory. It is an 'opt in' form that requires a parent signature in order for information to be included in the directory. The directory we produce is free to all families at the school. This form is how we collect information for that directory.
