Pirates and Treasure collection sheet

Our PTA had a pirate theme for this year. I created this monthly collection sheet for students to collect their box tops. Once all of the collection sheets were turned in, I put all of the sheets in a basket and randomly drew a winner. That student got to pick a prize from our "treasure box".
