Our PTSO has a VIP (Very Important Parents) Volunteer Sign Up Form that we use and I have sent it out to many parents over the past few months. I made mention that I was revising the form to better suit our needs for this school year and it is finally done. It is on legal sized paper this year and I think it is going to be even more effective that our last form. I would love to share it...it is created is MS Works so if you have that program I will email it to you...if you don't please email me your fax # as that will be the only other way I can send it. Due to the way I have formated the borders and page it will not convert to a Word document and still be legible. This email address is being protected from spambots. A couple of years ago we tried to conserve on paper and did both on one form very confusing for parents. We learned do two seperate. Some parents didn't bother turning to the other side even though it was stated on there (other side). Several didn't send in money just the form to join.

Separate membership and volunteer forms? I'll have to run this one by everyone...

On the other hand...

we try to conserve paper and keeping membership & volunteer info on one half sheet accomplishes that.

On the other other hand...perhaps doing a two-sided sheet...membership app. on one side, volunteer on the other with a check box... "no, I do not wish to join PTA at this time, but would still like to volunteer for the following activities." Any thoughts???

Forgot to mention that we send out our membership forms separately during the membership drive.

We send out a volunteer form at the beginning of the year. It is a two sided sheet with an introduction by the Pres & VP with their phone #'s and a listing of each committee including a place to check mark and a brief description of what is entailed. It also includes the time of year. On the other side there is a place for the parent's name etc including email address and area for comments. We list all the programs funded by the PTA and ask if there are any other ideas for projects they would like to see accomplished for the students. We get a good return and lots of good ideas. Having a description of the committees' responsibilities helps to show what time will be involved and whether it interests you or not.
