I did them myself. Bought folders at Walmart 10cents each. Had a square on front cover that fit a label nicely. I printed the correct size label with all school info and put on cover. Even had business card holder inside on both sides if needed.

Since this company just did some work for another organization I work with I thought I'd throw their site out there for you. Rather just make an information folder, this folder could be used by the teachers all year long for homework. (Nice vinyl folders - with printed logos/names, etc.)

They print custom folders for schools. There 1-color prices are pretty reasonable, IMO. This might be an option for you, they also let you customize the back and the folder flaps.
We put together folders with our contact numbers... a page that has all our fundraisers and dates for the entire year, plus contact info for chairperson of each event...
Blank paper for notes.. and a pen for writing anything down.
These are given out at the beginning of the year at meet the teacher night for kindergarten.
We use our 2 school colors for folders and put a label on the front that says PTO information
